You will need your Envelope # (member ID) for the registration - it can be found on your
contribution envelopes or by calling the church office (337) 478-3845. If you do not have
internet access, please contact the office for the paper version of the registration paperwork.
Phones and Communication: Flocknote is used as a primary means of communication with our
families. Unless we are notified differently, both parents will receive Flocknote texts and
emails. Emergency calls will be made to parents. If additional emergency contacts need to
be added, please do so during registration or contact the the SMdP Religious Education
office, Tootsie Wainwright. When entering phone information, you can use the same cell
number multiple times using a different drop down to indicate text number and emergency
number.
NOTE: Registration is complete only when all registration forms, fees, copy of birth certificate
(if not already on file) and official sacramental certificates (if applicable: Baptism, First
Eucharist, Confirmation) are received and on file in the SMdP Religious Education office. To
pay registration either wait 5 seconds after hitting submit on the registration form and you will
be linked to the VANCO site for payment or bring a check to the church office (337) 478-3845.
COST:
Registration Fee: 1 child = $40 / 2 children = $55 / 3 or more children = $70
Sacramental Fee per child in 2nd, 11th or RCIA: $20 each
Confirmation Retreat - each child in 11th grade: $150 - can be paid at a later date

Submission of this registration form is your authorization that you understand and agree to
cooperate with the St. Martin de Porres Catholic Church policies and requirements set forth in
the Parent Handbook, found at www.smdpcatholic.com. You hereby state: I give consent
for my family to participate in the following RE activities:
* Service projects, prayer services, retreats, and various teaching events which may
necessitate relocating my child(ren) between church facilities and/or St. John Elementary by
approved RE Staff or volunteers.
* Pictures of my child(ren) on parish website and RE Facebook page (no names will be used).
* Age-appropriate lessons on Christian morality and sexuality.
* Flocknote texts and/or emails to receive information on the St. Martin de Porres Catholic
Church Religious Education Program.
* My student will wear a mask to all RE classes and activities when public health officials have
mandated it or when the SMdP RE Staff requires it.
* My student may have their temperature taken before I depart when dropping them off. If
their temperature is 100.4 or higher, I will take them home.
* I will not bring my child to RE if they have been ill, used fever reducing medications, or
missed school in the previous 24 hours.
I understand that the permission hereby granted will remain in force unless rescinded in writing
BEFORE the actual event. I also confirm all contact information provided in this registration is
correct.

Instructions
SUBMIT BUTTON - When the entire form has been completed, there is a "Submit Form" button at the top and bottom of the form that can be clicked to send the registration. You may need to scroll to the top to see any messages related to missed items or confirmation of submission. There will be a 5 second delay and then you will be routed to the VANCO site to make payment of the fees.

		ALLERGY/SPCL ND - Enter any student allergies or special needs in the Student Remarks section of the form. If there are no allergies or special needs, enter the word None.

		FEES AND DONATIONS
		Tuition Due: $40 for 1 child; $55 for 2 children; $70 for 3 or more children

		Add 2nd Grade Fees: $20 Sacramental Fee per child in 2nd Grade

		Add 11th Grade Fees: $20 Sacramental Fee PLUS $150 Retreat Fee per child in 11th Grade ($150 fee may be paid at a later date if needed)

		OPTIONAL - Gifts from the Heart donations may be made upon completion of the registration process. Donations will be used to help defray the cost of religious education for those who need a helping hand or to provide additional learning resources in the SMdP RE Program.
