If you do not have internet access, please call Tootsie in the office online registration assistance. Phones and Communication: Flocknote is used as a primary means of communication with our families. Unless we are notified differently, both parents will receive Flocknote texts and emails. Emergency calls will be made to parents. If additional emergency contacts need to be added, please contact the SMdP Religious Education office, Tootsie Wainwright. NOTE: Registration is complete only when all registration forms, fees, copy of birth certificate (if not already on file) and official sacramental certificates (if applicable: Baptism and/or First Eucharist) are received and on file in the SMdP Religious Education office. To pay registration either wait 3 seconds after hitting submit on the registration form and you will be linked to the VANCO site for payment or bring a check to the church office (337) 478-3845. Submission of this registration form is your authorization that you understand and agree to cooperate with the St. Martin de Porres Catholic Church policies and requirements set forth in the Parent Handbook, found at www.smdpcatholic.com. You hereby state: I give consent for my family to participate in the following RE activities: 1) Service projects; prayer services; retreats; and various teaching events which may necessitate relocating my child(ren) between church facilities and/or St. John Elementary by approved RE Staff or volunteers. 2) Pictures of my child(ren) on the parish social media and print materials. 3) Age-appropriate lessons on Christian morality and sexuality. 4) Flocknote texts and/or emails will be sent to me to receive information on the St. Martin de Porres Catholic Church Religious Education Program. 5) My student will wear a mask to all RE classes and activities when public health officials have mandated it or when the SMdP RE Staff requires it. 6) My student may have their temperature taken before I depart when dropping them off. If their temperature is 100.4 or higher, I will take them home. 7) I will not bring my child to RE if they have been ill, used fever reducing medications, or missed school in the previous 24-hours due to illness. I understand that the permission hereby granted will remain in force unless rescinded in writing BEFORE the actual event. I also confirm all contact information provided in this registration is correct.
